Important Disclaimer
This framework describes behavioural patterns observed in emergency settings. It is NOT a diagnostic tool and does not diagnose personality disorders. We are not clinicians assessing mental health conditions. We are emergency responders classifying conduct that poses measurable risk to clinician safety and patient care. Classification of behaviour ≠ diagnosis of persons.
Foundations
Why are Narcissists what they are? The simple answer: a gross, deep inferiority complex and a hatred of the world. Before the tactics, understand the engine — the damaged ego, the cognitive dissonance, and the two modal states it flips between.
What drives narcissistic conduct?
The reasons generalise as a feeling of deprivation and being valueless, producing a weak, brittle and fragile Ego. This deficit is overcorrected by a person who remains conflicted by the fanatical confusion of who is the real me? — extreme cognitive dissonance in a brain that accommodates two diametrically opposed personae: the true self and the false self.
The false self despises the true self with its realistic view of actual defects, and builds a spoof, substitute, pretender personality. Beneath the cover, the Narcissist is incredibly needy and greedy. The mission here is not to micro-model the psyche — it is to help clinicians and normal folk recognise, manage and reduce the real-world threat. When confused: Remember: if a Narcissist is talking, he's lying.
Cognitive dissonance — the psychological discomfort of holding two contradictory beliefs simultaneously — was defined and named by psychologist Leon Festinger in his foundational work A Theory of Cognitive Dissonance (1957). The term is used throughout this framework in full acknowledgement of Festinger’s original contribution. As applied here, it describes the Narcissist’s irresolvable internal conflict between the False Self and the True Self — a specifically clinical application of the concept.
01.1 The Internal Mechanism
Everything starts with the internal drivers. This schematic illustrates the cognitive loop where perceived trauma and cognitive dissonance fuel deep-seated resentment, forcing the ego to adopt compensatory 'modes' to survive.
Figure 1: Narcissist Core Problem Drivers
An unresolved wound, and a cruel armour
Early in formative life, some event the person perceived as devastating — rejection, trauma, loss, neglect, or being over-valued then suddenly devalued — left a severe mental wound that was never resolved.
The corrective action is to bury that injury behind an armour against any future such event. That armour is ruthless selfishness. The person seeks to be accepted, valued and loved — but the armour makes that impossible.
01.2 The Narcissistic & Anti-Social Behavioural Spectrum
Narcissistic behavioural traits exist on a continuum, not as a binary. This spectrum ranges from adaptive self-confidence through to extreme manipulative conduct. Understanding where observed behaviour sits on this spectrum helps emergency responders assess risk and choose appropriate response strategies.
Figure 2: The Narcissistic & Anti-Social Behavioural Spectrum
This diagram illustrates behavioural patterns observed in emergency settings, not clinical diagnoses. The pathways show how different conduct types can escalate under specific triggers.
Most individuals exhibit some narcissistic traits under stress or in competitive contexts — this is normal and adaptive. Concern arises when these patterns become pervasive, persistent, and create measurable risk to others. At the extreme end, we observe: systematic exploitation, lack of empathy, predatory behaviour, and deliberate harm to emergency responders and genuine patients.
01.3 Narcissist lifestyle and behaviour
The Narcissist leads a negative and deceptive life, with little empathy or consideration for others. He is not easy to love or like. With few genuine friends, he often ends up alone. He is not emotionally self-sufficient and craves attention, reassurance and validation per the generalised behavioural cycle below:
Figure 3: Attention seeking / validation drivers
01.4 Nights are exhausting
The Narcissist's entire life is a lie. He suppresses truth daily in acts of grandeur of being strong, happy, intelligent, a winner, a success but behind the mask, he is miserable.
Figure 4: The false self meets the true self
By day, the Narcissist is constantly presenting an act or another or executing a bad, deceptive or malicious action. Beneath this act, two spirits are in conflict: the ruthless primitive “False self” with self-survival as its priority; and the silenced and numbed True Self. Dr James F. Masterson — American psychiatrist, founder of the Masterson Institute, and author of the seminal work The Real Self (1985) — terms this the “Real” Self which wraps the inconvenient and tortured conscience. His body of work on the True Self / False Self distinction in narcissistic and borderline personality remains a cornerstone of the field, and is acknowledged here with full credit. The Narcissist is extremely defensive of his core secrets of weakness and falsehood.
Acting out his daily roles and living in internal immoral / moral conflict burns so much energy that the only real relief is reaching the bastion of privacy.
Here, the mask can slip as his audience can no longer see him. His continuous hyper-vigilance can also relax in the safety of his lair.
However, Solitude is a double-edged sword. He gains recovery time and space, but he faces another crisis.
01.5 Why do Narcissists habitually call 999 Ambulances?
Figure 5: Attention seeking amplification mechanism
Already unhappy and with deeply embedded neediness, solitude to a Narcissist is terror, a trauma.
With a Narcissist's embedded neediness and desire for popularity, Solitude is loneliness and isolation.
Without friends, most often in the evening or the middle of the night, he feels entirely alone. Just then, a little voice said "Cheer up! Things could be worse." And sure enough, they got worse.
Solitude brings Reflection brings internal conflict and more cognitive dissonance.
The suppressed, subordinate True Self that fully knows the truth mercilessly attacks the normally dominant False Self's façade, deceptions and bad acts.
This is a conflict of delayed, disabled conscience coming alive and subordinated, muted voice meeting its oppressor. The dormant True Self comes online and active. The Intelligent thinking-brain now challenges the primitive caveman brain.
Unlike those he normally deceives, the True Self shares the same facts' Database. Now, he faces the one who he cannot bluff, confuse or lie his way out to escape. This is self-interrogation and self-prosecution. This brings no relief of inner guilt as the underlying core ego will not permit change or remedy. And so, he calls an Ambulance.
01.6 Ambulance arrives, what now?
Figure 6 — The truth or no truth, or nothing like the truth?
Will the natural risk taking, adrenaline-driven Narcissist come clean? I'm lonely, I'm unpleasant, I have no friends, I'm weak, I'm a total fake, I'm depressed … ?
No, the Narcissist must revert to type. He will now rise to the challenge, a low-risk joust with Paramedics. This is a Superiority, manipulation and abuse opportunity with little or no attached risk. The decision is a no brainer! Use his best skills, Lie! So he now has to weave a tapestry of illness, from well prepared and rehearsed Diagnosis-by-Google Symptomology.
If he fools the Paramedics, he will get to hospital. If that fails, he can verbally abuse them for free and attempt to provoke a conflict with no consequence. All now depends upon his ability to conceal the true facts of the matter that he is not ill and not an emergency. This is a very challenging mind-game.
He has constructed a series of defensive layers to hide the truth. Those closest to the schematic "truth" are the first defences to be used. Those furthest from the core are the most desperate and urgent when there is a fear that the truth will eventually out.
01.7 The Defense Layers: Secrecy & Concealment
This cycle of need does not operate in plain sight — it is protected by a sophisticated defensive architecture. Narcissists systematically conceal their true nature through multiple layers of obfuscation, denial, and misdirection. Each layer serves to deflect scrutiny, maintain the false self, and prevent exposure of the wounded core. Understanding this defensive structure is essential for clinicians who must penetrate these barriers to reach accurate assessment.
Figure 7: Narc Secrecy Layers — The Defensive Architecture
Initial Outer Defences, Layers and Relevance
The Narc Secrecy layers are the inner defences. Even before these are encountered, the accomplished Narcissist will often lay down a Charm-offensive as a smokescreen of sophistication, entitlement and assumed/projected credibility. This will morph into a web of deflection of any personal blame to others including an intrusively inquisitive Paramedic. A final overlapping layer is Reality Distortion coinciding with Obfuscation. None of the many defensive tactics is isolated or used alone. As to why this is so? This has little or no relevance to Acute Medicine. Emergencies are in current real-time not any long past-historic alleged or actual event.
The outermost layer: charisma, confidence, social grace. This facade is designed to attract, disarm, and create positive first impressions that mask underlying pathology.
Active defence: blame-shifting, Agenda-bending, topic-changing, minimization. When challenged, the Narcissist immediately redirects attention away from their behaviour onto external targets.
Deep layer: gaslighting, revision of history, creation of alternative narratives. The goal is to make observers question their own perception and memory of events.
Narcissists don't rely on a single tactic — they deploy overlapping, mutually-reinforcing defense mechanisms. When one layer is penetrated, another activates. In clinical settings, this manifests as resistance to assessment, inconsistent history-giving, and aggressive counter-accusations when pressed for truth. Expect evasion, expect misdirection, expect denial.
“Understanding the defensive architecture is the first step. The books map the case histories behind every layer.”

To Emergency Medicine, the cause matters not a jot
Emergency Medicine deals with emergencies — not social, psychiatric or personality issues. Nor is it society's duty to bow to every Narcissist demand. It is the Narcissist who must adapt — and they do not change for the better.
